Consent and Release Agreement - Participation *

In consideration of being allowed to participate in any activity or program at Design Hive, the undersigned, on his or her behalf, and on the behalf of the participant(s) identified, acknowledges, appreciates and agrees to the following conditions: I represent that I am the parent or legal guardian of the participant(s) named, or I have obtained permission from the parent/legal guardian of the participant(s) named below to execute this agreement on their behalf. I agree that the participant(s) named and I shall comply with all stated and customary terms, posted safety guidelines, rules, and verbal instructions as conditions for participation in any activity or program at Design Hive. I, for myself and the participant(s) named, and any representative, hereby release and hold harmless, Design Hive LLC., their affiliates, officers, members, agents, employees and other participants, and sponsoring agencies from and against any and all claims, injuries, liabilities or damages arising out of or related to our participation in any and all Design Hive programs and activities.

Consent and Release Agreement - Photo/Video Release *

I hereby agree to participate in the documentation of any performance, presentation, or other activity associated with Design Hive (DH), and authorize DH to record, rerecord and photograph my likeness, performance, voice, and artwork. BBCM may edit and adapt my participation as it deems appropriate. I further agree that the program and all of its elements, including all still photographs taken in connection with it, belong entirely and exclusively to DH, so that DH and its licensees and assigns may duplicate, distribute, broadcast exhibit or otherwise use or exploit the program, any portion of it, or related still photographs, without limitation or restriction throughout the world in perpetuity. If my participation includes music owned, composed or arranged by me, DH will have synchronization and performing rights so that the music can be used in the program. If my participation includes still photographs, footage, artwork or other materials owned by me, DH will have the right to use such materials in the program. I release DH and its licensees and assigns from any liability arising from my participation in the program, and authorize use of my name, likeness, voice performance, and biography for project purpose, including but not limited to program and institutional publicity, supplementary literary material, and other purposes. I waive the right to inspect or approve the photographs, footage, artwork or other materials owned by me before they may be duplicated, distributed, broadcast exhibited or used. I intend to be legally bound by this release. I am the parent or guardian of the minor(s) named, and I agree to the terms as provided above and am signing on the minor’s behalf. If in the future the minor voids this consent and release, I agree to indemnify DH and its licensees and assigns from any liability that may be incurred because of the minor’s action.
